Each of
these forms is developed as a PDF so that a new patient can download
them in their home, complete the form(s) and bring them in for the
first appointment. This eliminates the fee for mailing forms and
reduces the patient time in the office during the first visit.
Additionally, PUMC can
program the forms so that the new patient can complete the form(s)
on line and the submit them to your office. This allows you
to have a clean, typed form already on file before the
patient comes in.
